Client Paperwork & FAQ
-Client Paperwork
Please download, read, fill out and sign these two documents before coming in for your first appointment. The disclosure statement is required by law and simply states the way I work and some of the laws governing our work. The information sheet will provide me some background information about you which will help us in our work and will help me begin to get to know you. Please print and fill out one of these for each client (ie, each member of the family or couple).
What do I charge for a session?
I charge $175 for an initial evaluation session, which may last up to 75 minutes. I offer a sliding scale ranging from $110- $150 for our regular 60 minute sessions and $190-$225 for 90 minute sessions.
Do I take insurance?
I do not directly bill insurance, but I can provide a monthly statement for you to submit for reimbursement. I can't guarantee reimbursement, however, most insurance companies do reimburse for my services at the out-of-network provider rate. Clients are responsible for contacting their insurance to verify coverage.
What is my billing policy?
I can accept payment from all major credit/debit cards. If you are going to pay by check, please come to each session with a check already made out to Sheryl Kent for the appropriate amount according to session length. This will allow us to spend our time together focused on the important work of therapy, not check writing! I expect payment at the time of each session and do not bill for sessions.
How long can I expect to be in therapy?
The answer to this question is different for everyone. In fact, you are an essential part of determining this answer. At the start of therapy we will collaboratively develop treatment goals. We will periodically discuss movement towards these goals and I encourage to reflect on our progress yourself. Some people terminate when these goals are met, others discover new goals along the way.
How often should I come?
Again, this is a personal choice. Some people prefer to come weekly, others every other week and still others determine another unique rhythm. The pacing may be impacted by factors such as level of crisis, a personal deadline, or a desire to move slowly and take time to integrate. Some clients come intensely for a while and then take a break, returning after a few months. I suggest you listen to yourself about what makes sense for you.
